Pre-Screening Process

Complete the pre screening form so we can better understand your needs.

Once submitted, your form will be reviewed. If your concerns align with the services offered, you will receive a link to schedule a free 15 minute consultation. If not, you will still receive a response with next steps or referral options when possible.

Submitting the form does not guarantee services or an appointment.

Our Client Onboarding Process

1. Screening Request

Complete the pre-therapy screening form so I can review your needs and confirm fit.

2. Schedule a 15-Minute Consultation

If your needs align with our services, a brief phone or virtual consultation will be scheduled to discuss next steps.

3. Intake Assessment

What to Expect During Your 60 minute

Intake Biopsychosocial Assessment

The first session is all about connection and clarity. It’s our time to begin building trust, review key policies and consent forms, and talk through what you’re hoping to get out of therapy.

Before we move into ongoing sessions, we’ll complete an initial biopsychosocial assessment, which is a "whole person" evaluation that gives me a deeper understanding of your lived experiences. This includes:

  • Biological: your medical and physical health history

  • Psychological: your mental and emotional well-being

  • Social: your relationships, circle, work or school life, and support systems

This step helps me create a strong foundation for our work. It gives me insight into where you’ve been, where you are now, and how I can best support you as you move forward.

4. Paperwork + Portal Setup

You will complete consent forms and intake paperwork in a HIPAA-compliant portal before your first session.

5. Ongoing Therapy + Support

Ongoing sessions are centered on building a strong therapeutic relationship, collaborative goal setting, practical tools, and lasting change in daily life.
